A pre-assessment will be undertaken prior to your commencing with AQ Health.

Programs and services can be accessed under the Commonwealth Home Support Programme (CHSP), Home Care Packages (HCP), and Chronic Disease Management (CDM) program.

A GP referral is needed to access the CDM program. For CHSP and HCP access you will need an aged care assessment and may be asked to contribute an income tested care fee to AQ. Your income tested care fee is calculated by Services Australia (refer to myagedcare.gov.au). AQ can support you through the process to obtain a My Aged Care referral as needed.

You can pay for AQ services privately, including accessing private health insurance rebates.
